A history of enabling people to remain living happily at home

We have been enabling people to continue to live independently at home for more than 25 years.

We are proud to have supported over 40,000 Australians live happily at home and stay connected to their community.

 

Our Milestones
1994
  • Established as West Care Linkages with a staff of three and one office in Melbourne’s inner west with the aim of preventing the premature or inappropriate admission of frail aged and younger disabled persons to residential care (nursing homes and hostels).
2002
  • Relaunched as Care Connect Limited.
  • Expanded services into NSW.
  • 2,210 clients supported with services.
  • Spoke at United Nations World Assembly.
2003
  • Secured NSW government ‘ComPacks’ funding to support people transitioning home from hospital.
  • Finalist in the Equity Trustee’s CEO Innovation Award recognising excellence in the not-for-profit sector.
2004
  • Expanded services into QLD.
  • Became ISO 9001 certified.
2005
  • Created ‘Commitment to Indigenous Communities’ statement to support and enhance services to local Indigenous communities.
2006
  • Received funding for Indigenous program.
  • 3,810 clients supported by 161 staff across 13 offices in four states.
  • 158 different types of services provided.
2007
  • Established a Consumer Voice Committee.
2008
  • Opened respite retreats in Rosebud and Daylesford.
  • Became the largest provider of the ComPacks program in NSW.
2009
  • Established lesbian, gay, bisexual, transgender or intersex tailored programs.
2010
  • Became Australia’s first community aged care service for frail older lesbian, gay, bisexual, transgender or intersex people in QLD.
2011
  • Expanded service delivery from two to five regions in Queensland.
  • Awarded program funding for humanitarian entrants.
2012
  • Launched ‘My Life, My Choice, My Way’, a person-centred service model focused on client choice and independence.
  • 7,300 clients supported by 300 staff across Vic, NSW and Qld.
  • Successful trial of innovative iCareConnect program providing greater services to people remotely via the Internet.
  • Advocated for a National Disability Insurance Scheme.
2013
  • Expanded transitional and disability services in NSW.
  • Diverse Community Care merged with Care Connect.
  • Secured HomeShare funding for Melbourne’s North West region.
  • Appointed as the Veterans’ Home Care Assessment Agency for the Loddon Mallee region of Victoria.
  • Supported clients speaking 37 different languages.
  • Invited to present at 16 industry conferences nationally.
2014
  • 12,000 clients supported by 337 staff across 15 offices.
  • Celebrated 20 years of connecting.
  • Received Victorian Human Services Standards Certification.
2015
  • Achieved 100% compliance against all Standards in quality audits from the Australian Aged Care Quality Agency.
  • Expanded iCareConnect across NSW and Qld.
  • Recognised with NSW Disability Services Standards Certification.
2016
  • Launched new brand and vision.
  • Implemented ambitious new strategy to meet the challenges of a new era in care and support services.
  • Received ‘Best Aged Care Software Development and/or Deployment ITAC Award’ at the Information Technology in Aged Care Conference.
